Continuing Your Physical Therapy Care at Oakridge
If you have been receiving physical therapy services at Oakridge Wellness Center, we understand that transitions can raise questions. Our goal is to make this process smooth, clear, and supportive for you.
Coastal Care Partners Physical Therapy is now the provider of services at Oakridge Wellness Center. Because we are a new and independent practice, patients must formally establish care with Coastal Care Partners before continuing therapy.
We are committed to ensuring continuity of care through a thoughtful evaluation and personalized treatment plan.
What This Means for You
Even if you have been actively attending therapy sessions, you will need to:
- Schedule a new patient evaluation with Coastal Care
- Complete standard new patient registration forms
- Provide a physician referral if required by your insurance plan
This is not a reflection of your prior care — it is simply part of establishing care with a new provider.
Step 1: Schedule Your Initial Evaluation
Your first visit with Coastal Care Partners will be a comprehensive evaluation.
During this appointment, your therapist will:
- Review your medical history and current diagnosis
- Perform a detailed physical assessment
- Discuss your goals and expectations
- Establish a clear plan of care and recommended visit frequency
This ensures we begin treatment with full understanding of your needs and priorities.
Step 2: Complete Registration & Insurance Verification
As with any new healthcare provider, we require standard intake documentation. Our team will guide you through:
- New patient forms
- Insurance verification (if applicable)
- Determining whether an updated referral is required
If you have questions about insurance coverage, we are happy to assist you.
Step 3: Prior Therapy Records (Optional but Helpful)
Your medical records from your previous provider are not automatically transferred.
If you would like your prior therapy notes included in your new evaluation, you may request them directly from your previous provider and bring them to your first visit.
Helpful items may include:
- Prior evaluations
- Plan of care
- Progress notes
- Surgical reports or imaging summaries
While helpful, these are not required to begin care.
