Step 1: Verify Membership
Physical therapy services at Oakridge Wellness Center are available exclusively to members of The Landings Golf & Athletic Club.
Before scheduling online, you will be asked to provide your Landings member number to confirm eligibility.
Step 2: Schedule Your Initial Evaluation
Every patient begins with a comprehensive one-on-one evaluation.
During this visit, your therapist will:
- Listen carefully to your concerns and goals
- Review relevant medical history
- Assess strength, mobility, balance, and movement patterns
- Identify contributing factors to pain or limitation
- Develop a customized plan of care
This evaluation ensures your treatment plan is specific, measurable, and aligned with your personal goals.
Step 3: Complete New Patient Registration
As with any healthcare provider, new patient intake documentation is required.
Our team will assist you with:
- Contact and medical history forms
- Insurance verification (if applicable)
- Determining whether a physician referral is required

Step 4: Begin Your Personalized Plan of Care
After your evaluation, your therapist will outline:
- Recommended visit frequency
