Physical Therapy Services Transition

Continuity of Care. A Seamless Transition.

We want to personally update you regarding an important transition in Physical Therapy services at Oakridge.

The current provider will conclude services on Friday, March 13th.

Beginning Monday, March 16th, Coastal Care Partners will begin offering Physical Therapy services to ensure continuity of care for our community.

Our goal is simple:

To make this transition smooth, reassuring, and uninterrupted for every patient.

Temporary Location & Schedule

From March 16th through April 3rd, Physical Therapy services will be offered temporarily inside the Oakridge Wellness Center. (Please ask at the check-in desk for location)

During this transition period, hours will be:

Monday–Friday

10:00 AM – 6:00 PM

IMPORTANT INFORMATION

Due to temporary space limitations within the Wellness Center, this schedule will be reduced during the transition.

To ensure the highest level of care and continuity, we will be prioritizing post-operative recovery patients and individuals who require time-sensitive therapy during this initial period.

We appreciate your understanding as we work to expand back into the main Physical Therapy space.

We anticipate returning to the primary Physical Therapy suite on:

Monday, April 6th

At that time, normal hours will resume:

Monday–Friday

9:00 AM – 6:00 PM

Important Information for Current Patients

If you are currently receiving care from the current provider, we look forward to continuing your therapy without interruption.

Because Coastal Care Partners will be considered a new provider under insurance guidelines, patients will need to:

• Call to transition care
• Establish services under Coastal Care Partners

This step is necessary to ensure proper insurance billing and compliance.
